Position Summary The Clinical Liaison serves as the first point of contact for hospitalized patients and their families who may benefit from Ennoble Care’s hospice and palliative care services. This role is responsible for educating patients, families, and healthcare partners about Ennoble Care’s programs, supporting informed decision-making, and facilitating smooth transitions of care. The Clinical Liaison collaborates closely with physicians, interdisciplinary team members, and acute care partners to develop and initiate individualized, goal-aligned care plans. Key Responsibilities Serve as the primary liaison between Ennoble Care and acute care facilities, identifying patients appropriate for hospice or palliative care services. Introduce and educate patients and families on Ennoble Care’s hospice and palliative care philosophy, services, and benefits. Provide emotional support and guidance to patients and families during care transitions. Collaborate with patients, families, physicians, and the interdisciplinary team to develop care plans aligned with patient goals and wishes.
