Nice to Have
Master of Divinity (MDiv) or equivalent theological degree, Experience providing spiritual counseling in hospice, palliative care, hospital, or healthcare settings, Experience working with individuals and families facing serious illness, death, and bereavement, Completion of Clinical Pastoral Education (CPE), Ordination or ecclesiastical endorsement by a recognized faith group, where applicable
What You'll Do.
Provides spiritual counseling services as a core hospice service in accordance with the Medicare Hospice Conditions of Participation (42 CFR §418. 64)
assessing and addressing the spiritual needs of hospice patients and families related to the terminal illness
promoting patient well‑being
and dignity throughout the dying process
Provides spiritual assessment and appropriate intervention as needed for hospice clients and families in keeping with the clients'/families' beliefs
Makes routine visits to patients that have requested clergy visits
Identifies patient's individual spiritual needs and takes independent action within the confines of professional role
Follows the hospice philosophy
Provides spiritual counseling regarding spiritual or emotional difficulties for patients and their families
Helps develop individualized patient care plans developed and implemented according to spiritualotional needs of the patient/family
Provides pertinent information to patient/family members regarding spiritual issues
Maintains accurate and concise hospice records for all patient activity which should be completed within specified time frames
Assists or facilitates in bereavement follow-up (visit
and support groups) as requested
Attends hospice Inter-Disciplinary Team (IDT) meetings as requested
Assists with projects and activities such as the annual hospice memorial service and as requested
May conduct or arrange for funeral or memorial services upon request
at their discretion and on a case-by-case basis
Shall comply with Title VI of Civil Rights Act
Comply with Hospice's personnel policies and health standards and will complete any mandatory education as assigned by Hospice
Performs other duties and responsibilities as assigned
