Other duties may be assigned. * Directly oversees all aspects of admission process for all market skilled nursing centers and market admissions associates. * Develops and implements training programs and professional development plans to educate Admissions Coordinators, Admissions Assistants and other team members on skilled nursing admission guidelines and processes. * Participate in Nursing Center QAPI process. * Responsible for meeting established goals related to admissions for market skilled nursing centers, referral source relationship development, outreach program implementation and other quality metrics established. * Communicates with organization personnel, especially designated market Patient Transition Directors regarding patient transitions. * Develops and coordinates admissions associates in strategic outreach activity in market in order to facilitate referrals to skilled nursing centers. * Performs strategic outreach events in market in order to increase community awareness and facilitate referrals to skilled nursing centers. * Reviews monthly analytic data. Tracks information to determine market trends, and develops action plans based on data relative to key performance indicators. * Attend and participate in continuing educational programs to keep abreast of changes in your field as well as to maintain current license/certification, as required. * Attend and participate in mandatory in-services. * Honor patient’s rights to fair and equitable treatment, self-determination, individuality, privacy, proper
