Full Job Description

Lighthouse Behavioral Health Solutions (LBHS) offers a full continuum of care, including outpatient, intensive outpatient, partial hospitalization, residential treatment, psychiatric services, and medication‑assisted treatment. Position: Case Manager (CDCA) Job Summary: Provide support services and foster community by assessing the needs of their clients and developing and implementing a comprehensive plan to address those needs. This position reports to a Case Management Supervisor. Duties and Responsibilities: Coordinates and provides safe, timely, effective, efficient, equitable, culturally sensitive, and client- centered care. For SUD Clients: Handles case assignments, assesses barriers to recovery in the various life domains, may draft SUD case management-specific service plan, review progress in case management goals. For MH Clients: Handles case assignments, assesses barriers to recovery in the various life domains, coordinates mental health treatment planning including goals for CPST services, provides input, and works collaboratively with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan. Is familiar with the diagnostic assessment completed for the clients served, which includes a review of diagnosis, identification of current symptoms, risk, functioning, identified needs in the client's life domains shown as Z-Codes to determine eligibility for Case Management or CPST services and the level of care needed. Works collaboratively following up with observations with the clinical counselor, psychiatrist, and/or nurse practitioner in the development of the treatment plan.
