What You'll Achieve.
Facilitating their successful transition to community living; Ensure that inmates with Behavioral Health needs are being identified; Ensure inmates have access to BH services upon release; Matching available resources with assessed needs

Maintain a consistent, highly visible in-person presence within the prison (on average 75% of time), Travel to and from consumer locations, providers, hospitals, etc., Potential for exposure to Hepatitis B, Access to training and voluntary vaccination against Hepatitis provided
What They're Looking For.
Must Have
Two years of experience as a Caseworker for Lancaster, A bachelor's degree in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences and three years of professional social casework experience in a public or private social services, Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other related social sciences, Pennsylvania Childline Child Abuse History Clearance, Acceptable-PA State Police Criminal Record Clearance, Acceptable FBI fingerprint clearance, Acceptable Drug screening, PA Driver's License and Acceptable driving history
What You'll Do.
Conduct intake assessments
Determine eligibility for services
Work with incarcerated adults
Assist individuals transitioning back
Conduct intake interviews
Gather information concerning consumer
Complete referral forms
Develop supportive relationships
Monitor consumer progress
Maintain knowledge of community resources
Advocate on behalf of consumer
Complete paperwork demands
Coordinate service plans
Develop consumer records
Determine financial liability assessments
Participate in staff meetings
Attend training sessions
Coordinate interagency meetings
Provide own transportation
Function as County's primary resource
Provide clinical consultation
Serve as Agency liaison
Exercise supervisory decision making

Work collaboratively with Lancaster County Prison; Work collaboratively with Lancaster County Adult Probation & Parole; Work collaboratively with Lancaster County Court of Common Pleas; Work collaboratively with Lancaster County Speciality Courts; Work collaboratively with the Lancaster County Drug & Alcohol Commission; Work collaboratively with the Re-entry Management Organization; Collaborative working relationships with correctional staff; Collaborative working relationships with medical staff; Collaborative working relationships with clinical staff; Collaborative working relationships with reentry staff; Collaborative relationship with local prison; Collaborative relationship with local hospital; Establish and maintain relationships with other County departments; Establish and maintain relationships with the Courts; Establish and maintain relationships with the Re-entry Management Organization; Engages in multi-systemic forums
