Must Have
High School Diploma or GED required, Valid driver's license required, Colorado drivers license within 30-days of hire
Nice to Have
Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred, Bilingual or multilingual skills preferred, 1-2 years experience with mental health challenges, Experience with individuals experiencing homelessness, Experience with mental health challenges, Experience with substance use disorders
What You'll Do.
Develop and implement care plans
Coordinate services and resources
Monitor client progress
Provide advocacy and support
Facilitate referrals to other services
Maintain case documentation
Maintain data collection
Deliver outreach services
Facilitate linkages to mainstream services
Facilitate linkages to housing
Provide crisis intervention
Manage caseload of clients
Maintain positive relationships with community partners
Maintain positive relationships with health providers
Support clients with wrap-around care
Ensure consistent communication

Act as liaison between management
