Bachelor's degree in psychology, Bachelor's degree in Social Work, Bachelor's degree in Human Services, Bachelor's degree in Criminal Justice, 1-2 years experience with SPMI population, Experience with homelessness, Experience with mental health challenges, Experience with substance use disorders, Knowledge of local housing resources, Knowledge of benefits programs, Knowledge of social service agencies, Bilingual or multilingual skills
What You'll Do.
Monitor client progress
Maintain case documentation
Maintain data collection
Provide client support
Deliver outreach services
Facilitate referrals to services
Facilitate linkages to housing
Provide crisis intervention
Provide face-to-face services
Assist clients in defining needs
Inform clients of services
Help clients access goods
