Must Have
BSN upon hire, 3 years of experience in an acute care hospital setting, Licensed Registered Nurse in good standing with the State of Michigan, BLS certification required by completion of core orientation
Nice to Have
Master's degree preferred, Case Management Certification preferred
What You'll Do.
Implement patient care
Coordinate patient care
Monitor patient caseload
Ensure appropriate care
Serve as clinical resource
Collaborate with interdisciplinary team
Identify patients at risk
Perform further assessment
Initiate transition care plans
Manage patient progress
Document according to procedure
Round daily on patients
Identify readmissions
Prevent further readmissions
Communicate plan to team
Ensure smooth transition of care
Drive multidisciplinary team rounds
Document transitional planning
Enact transitional plan
Identify post discharge needs
Evaluate patient need for resources
Ensure effective healthcare management
Deliver transitional services
Assess eligibility for programs
Assist patient with paperwork
Collaborate with Patient Financial Counselors
