Lifting of 20 pounds maximum, with frequent lifting and/or carrying of items weighing up to 10 pounds, or requires significant periods of walking or standing., Normal tasks may involve exposure to blood, bodily fluids, or tissue., May involve 24-hour on-call responsibilities., Reliable transportation for frequent travel.

What They're Looking For.

Must Have

Bachelor’s degree in Social Work, Psychology, Sociology, Human Services, or related field as approved by Medicaid, or as required by applicable funding source., Minimum of zero year’s experience in case management/service coordination with a Bachelor’s degree., Working knowledge of mental and physical characteristics of assigned population group (e.g., growth and development), human behavior, treatment/service options, and related issues., Working knowledge of SCS programs and services, policy, and procedure., Working knowledge of applicable regulations and licensure standards., Good interpersonal skills and written and oral communication skills.

What You'll Do.

Conduct comprehensive assessment of client needs.

Participate in development of client’s service plan.

Arrange for delivery of needed services.

Assist client in accessing needed services.

Monitor client’s progress.

Perform advocacy activities on behalf of client.

Prepare and maintain case records.

Provide case consultation.

Communicate with supervisor about case management issues.

Provide psychosocial education to clients.

Perform crisis assistance.

How You'll Work.

Team & Collaboration

Communicate with supervisor about issues related to case management, resourcing, service collaboration, and development of new resources.; Provide case consultation (i.e., consulting with service providers/collaterals in determining client’s status and progress).
