What They're Looking For.
Must Have
High School Diploma or GED required, Valid driver's license required
Nice to Have
Bachelor's degree in Psychology, Social Work, Human Services, Criminal Justice preferred, Bilingual or multilingual skills preferred, Knowledge of local housing resources, benefits programs, and social service agencies preferred, Experience working with individuals experiencing homelessness, mental health challenges, or substance use disorders preferred, Experience working with a diverse population who are experiencing mental health challenges, particularly those individuals diagnosed with serious and persistent mental illness (SPMI) preferred
What You'll Do.
Develop and implement care plans
Coordinate services and resources
Monitor client progress
Provide advocacy and support
Facilitate referrals to other services
Maintain case documentation
Deliver outreach services
Facilitate referrals and linkages to mainstream services and
Provide crisis intervention
Manage a caseload of clients with high-service utilization
Maintain positive and co-working relationships with community partners
Support clients with a wrap-around care management model
Ensure consistent communication
Support clients through crisis or disengagement
Act as a liaison between internal program management
Respond promptly to crises involving residents
Provide de-escalation
Provide appropriate referrals
Work with individuals to develop and implement crisis
Maintain positive relationships
Track client engagement
Track client outcomes
Focus on individual client outcomes
Accompany residents to appointments
Accompany residents to meetings
Accompany residents to other necessary engagements
Provide support and advocacy
Assess mental health resource needs
Build rapport with individuals
Maintain on-going professional relationships with individuals

Assist clients in applying for and securing benefits
Facilitate referrals to external agencies
Coordinate care with other service providers
Contribute to enhancing and ensuring safety
