Must Have
Valid driver's license required, High School Diploma or GED required, Colorado drivers license within 30-days of hire
Nice to Have
Bachelor’s degree in Psychology, Social Work, Human Services, Criminal Justice preferred, Bilingual or multilingual skills preferred, 1-2 years experience with mental health challenges, Experience with individuals experiencing homelessness, Experience with substance use disorders

## Description The mission of the Colorado Coalition for the Homeless is to work collaboratively toward the prevention of homelessness and the creation of lasting solutions for homeless and at-risk families, children, and individuals throughout Colorado. The Coalition advocates for and provides a continuum of housing and a variety of services to improve the health, well-being, and stability of those it serves. The Case Manager I works collaboratively alongside case managers, supervisors, integrated health team members, and community-based partners across the organization to promote long term solutions for families, children, and individuals experiencing homelessness or who have formerly experienced homelessness. This role is responsible for developing and implementing care plans, coordinating services and resources, monitoring client progress, providing advocacy and support, facilitating referrals to other services, and maintaining case documentation and data collection.
