What You'll Do.

Assume responsibility and accountability for a group of patients/clients

Engage with individuals to assess and prioritizes person’s served treatment and service needs

Develops and implements plan of care and collaborates with the clinical team to assure a uniform plan of care

Provides input to the interdisciplinary treatment plan

Serves as a liaison between community resources
