Bachelor's degree in related field of study required, Competency in co-occurring disorders, including ability to engage, screen, deescalate and coordinate care, Two years mental health experience, to be able to independently assess person’s served needs and to implement and evaluate effective plans of care and acquire and effectively utilize knowledge of policies and procedures
What You'll Do.
Assume responsibility and accountability for a group of patients/clients
Direct care to patients/clients via the development of the treatment approach including specific discipline interventions
Ensure that appropriate level of care is determined
Confer with the family referral source and the insurance company as to the status of the patient throughout treatment
Engage with individuals to assess and prioritizes person’s served treatment and service needs
Develops and implements plan of care and collaborates with the clinical team to assure a uniform plan of care
Provides input to the interdisciplinary treatment plan
functioning as a team member
Establishes therapeutic relationship with assigned person’s served including engaging
Serves as a liaison between community resources
and hospital- or clinic-based providers of care
Responsible to document and report all necessary information according to established policies and procedures
