What They're Looking For.
Must Have
Qualification in Social Work, Psychology, Welfare, Community Services or similar, Demonstrated experience in case management, particularly with families and children, is essential., Ability to engage hard-to-reach families and build trust, Current driver’s licence and willingness to travel, Current Driver’s Licence., First Aid Certificate., national police history check, qualifications, referee checks, clearance to work with children/vulnerable people
Nice to Have
Knowledge of issues like domestic violence, mental health, and child development is highly valued.
What You'll Do.
Deliver intensive case management to families with complex needs
Conduct home visits and provide outreach-based support
Undertake comprehensive assessments and develop tailored case plans
Support parents to strengthen parenting capacity and family functioning
Identify and respond to risks relating to child safety and wellbeing
Coordinate with internal and external services to deliver integrated support
Maintain accurate case notes
and compliance documentation
including occasional after-hours support
