What You'll Do.
Manage care of individuals in crisis
Facilitate admission intakes
Develop treatment plans
Develop discharge plans
Assist in legal process
Initiate personalized care plans
Coordinate care with treatment team
Coordinate care with families
Coordinate care with providers
Coordinate care with referral sources
Serve as ambassador to community partners
Develop and implement treatment plan
Complete signed treatment plan
Review treatment plan with individual
Gather collateral from individual
Gather collateral from involved parties
Coordinate care for patients
Identify service gaps
Make recommendations for discharge
Communicate with individuals
Maintain clinic record
Document activities performed
