Must Have
Bachelor of Arts in behavioral health, 1 year relevant post degree, Associate’s degree in direct care, 2 years relevant post degree, High school degree, Ability to use lived experience, Valid driver's license, Acceptable driving record, Independent thinking and good judgment, Analyze work, set goals, develop plans, Utilize time effectively and efficiently
Nice to Have
Complete peer certification training within one year
What You'll Do.
Engage consumers in discharge planning
Assist with transition to community
Provide post-discharge follow-up
Support consumer with community integration
Conduct assessments within correctional facilities
Attend court hearings
Advocate for persons served
Provide weekly face to face contact
Utilize treatment plan goals
Provide linkages across needs
Follow up for resources
Transport clients for identified goal needs
Maintain all documentation
Monitor treatment planning
Monitor discharge planning
Complete all required documentation
Adhere to flexible schedule
Demonstrate competent written communication
Demonstrate competent verbal communication
Meet obligations of the program
Follow program policies and procedures
Educate self on issues
Operate conscientiously in accordance with program budget
Perform other related duties
