Must Have
Active BLS certification, Valid Ohio driver’s license, Proof of auto insurance, Pass motor vehicle background inspection, Pass insurance eligibility, Pass driving qualifications, Pass training set forth by Nationwide Children’s Hospital, Maintain qualification of insurance guidelines
Nice to Have
Bachelor’s degree with background in health care, public health, or related clinical field, 2 years of experience working in healthcare in a patient facing role
What You'll Do.
Navigate and access community services
Support population health programs
Arrange or connect patients to services
Identify and nurture relationships with local agencies
Engage and motivate members in case management programs
Assist with development of family-centered care
Collect information for assessments
Refer onward when criteria met
Provide personalized navigation support
Connect families to resources
Manage and monitor transitions between settings
Perform outreach to providers
Develop patient and family self-management skills
Answer incoming telephone calls
Schedule appointments
Assist members to resolve immediate needs
Manage administrative functions
Prepare and maintain records
Participate in orientation and continuing education
