Bachelor’s degree with background in health care, public health, or related clinical field, 2 years of experience working in healthcare in a patient facing role
What You'll Do.
Navigate and access community services
Provide support through population health programs
Arrange for or connect patients to needed services
and nurture relationships with local agencies
Engage and motivate members in case management programs
Collect information for assessments
Refer onward when criteria met
Provide personalized navigation support
Connect families to resources
Manage and monitor transitions between settings
Perform outreach to PCP/POC’s
Develop patients and family’s self-management skills
Answer incoming telephone calls
Schedule appointments
Resolve immediate needs
Manage administrative functions
Prepare and maintain records
Participate in orientation and continuing education
