What You'll Do.
Navigate and access community services
Provide support through population health programs
Arrange or connect patients to needed services
and nurture relationships with local agencies
Engage and motivate members in case management programs
Collect information for assessments
Refer onward when criteria met
Provide personalized navigation support
Connect families to resources
Accommodate cultural and linguistic needs
Manage and monitor transitions between settings
Perform outreach to PCP/POC’s
Develop patients and family’s self-management skills
Answer incoming telephone calls
Schedule appointments
Assist members to resolve immediate needs
Manage administrative functions
Prepare and maintain records and case files
Participate in orientation and continuing education

Facilitates communication and collaboration amongst the healthcare team; Ability to work within a multidiscipline matrix
