What You'll Do.
Engage and motivate members
Assist with family-centered care development
Collect information for assessments
Refer onward when criteria met
Facilitate communication and collaboration
Provide personalized navigation support
Connect families to resources
Accommodate cultural and linguistic needs
Manage and monitor transitions
Provide follow-up across continuum
Perform outreach to providers
Research and facilitate referral
Develop self-management skills
Answer incoming telephone calls
Schedule appointments
Assist members to resolve needs
Manage administrative functions
Prepare and maintain records
Document clients' information
Document services provided
