What You'll Do.
Coordinate client engagement through appointments
and meetings in collaboration with the treatment team.
Maintain communication with referral sources and outside providers through consistent
professional interactions
and documenting contacts during admission and throughout treatment.
Coordinate transportation needs for clients.
Provide individual case coordination sessions.
Assist in developing aftercare plans
including referrals and addressing outside provider needs.
Support clinical services by facilitating groups and encouraging client engagement in the program.
