Must Have
Australian Drivers Licence, 2 years’ experience in Case management in home and community based settings
Nice to Have
Qualifications in Aged Care, Health, Community Services or related discipline, experience in building professional relationships with customers, their supporters and external service networks, demonstrating an understanding of My Aged Care, Holding similar values and principles associated with the provision of care for older adults and Each service principles, understanding and knowledge of the Aged Care Quality Standards, Community Home Care / Support at Home
What You'll Do.
Providing Older Adults
their families and carers with access to responsive care and case management
ensuring the customers receive services and support tailored to their needs and requirements
liaise with older persons
either on phone or in their home
to follow up on their care requirements
conversations will revolve around their goals to achieve happy
fulfilled lives whilst remaining independent in their own homes
Utilising your case management skills to undertake initial and ongoing comprehensive assessments and care planning to support clients at home
Managing the delivery of services to support the needs identified (psychosocial
physical and vulnerabilities) within the customer’s individualised budget
Drawing upon and building on relationships with colleagues
services networks and a range of community agencies to support the program and customers
