What You'll Do.

Engages with individuals receiving care management services

Adheres to all deadlines as outlined by the service definition

Assists the individual supported to direct the planning process

Provides education regarding services to all parties

Coordinates care team meetings with the individual

Coordinates appropriate referrals to community resources

Manages transitions of care for people transitioning

Communicates effectively with individuals utilizing person-centered practices

Communicates effectively with external stakeholders

Educates individuals/families on methodology for budget development

Establishes collaborative relationships with community stakeholders

Identifies and provides emergency crisis response

Review data to identify and determine appropriateness for services

Maintain trainings as required and requested

Complete all other relevant responsibilities as assigned
