Must Have

Must be 18 years of age or older, Be a person with lived experience with a behavioral health condition and a Certified Peer Support Specialist, A parent or guardian of an individual with a behavioral health condition and has at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual, Has two years of paid experience performing the types of functions listed in the job description, with at least one year of paid experience working directly with the Tailored Care Management eligible population, High School Diploma (Required), Drivers License (Valid) - USA, Direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (parent/guardian cannot serve as an extender for their family member) | 2 Years | Required, Paid experience performing the types of functions listed in the job description, with at least one year of paid experience working directly with the Tailored Care Management eligible population | 2 Years | Required

What You'll Do.

Perform general outreach

Provide services and supports to emphasize personal safety

Engage in various skill-building activities

Assist in the development of goals in the Plan of Care

Coordinate services/appointments

Provide services in various community settings

Link individuals to appropriate community-based services

Facilitate person-centered

positive relationships

Assist with self-help

Document encounters and contacts made

Attend regular staff meetings

Maintain all required training

Maintain certification in all agency

and federal training requirements

Assist new staff and/or current staff with orientation

Demonstrate knowledge of and comply with all agency policies

Follow service definition guidelines

Complete all other relevant responsibilities assigned

How You'll Work.

Team & Collaboration

In conjunction with the care management team, engage in various skill-building activities; Convey observations and key issues to supervisor on an on-going basis
