What They're Looking For.
Must Have
Active, unrestricted clinical certification or licensure (e.g., Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), or comparable credential), Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting, Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person, Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, Ability to pass a background check, Valid, unencumbered driver’s license, Consistent, reliable transportation, Current auto insurance
Nice to Have
Experience with Ennoble Care’s EMR, Knowledge of healthcare terminology and care coordination principles, Proficiency with Microsoft 365, including Outlook, Word, and Excel

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Leesburg, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
