Must Have
Active, unrestricted clinical certification or licensure, Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN), 1 year of patient-facing experience, Comfortable communicating proactively and professionally, Microsoft 365, Outlook, Word, Excel, Valid, unencumbered driver’s license, Consistent, reliable transportation, Current auto insurance, Ability to pass a background check

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Front Royal, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support. KEY RESPONSIBILITIES Coordinate care for long-term care residents using Ennoble Care’s EMR Maintain regular, proactive, on-site engagement with residents to support care plans, follow-ups, and care transitions Collaborate closely with Ennoble Care providers, within the long-term care setting Serve as a key liaison between providers, facility staff, residents, and families Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner Accurately document patient interactions and care
