What They're Looking For.
Must Have
Active, unrestricted clinical certification or licensure, such as: Patient Care Technician (PCT), Certified Nursing Assistant (CNA), Medical Assistant (MA) / Certified Medical Assistant (CMA), Licensed Practical Nurse (LPN), Registered Nurse (RN) or comparable credential, Minimum 1 year of patient-facing experience in a long-term care, skilled nursing, or similar clinical setting, Ability to work full-time, Monday through Friday, 8:00 a.m. to 4:30 p.m. EST, (or thereabouts, contingent on facility hours) in an on-site role, based in the Reston, Virginia area., Ability to pass a background check and drug screen

What You'll Do.
Coordinate care for long-term care residents using Ennoble Care’s EMR
on-site engagement with residents to support care plans
Communicate patient needs
and urgent concerns to the clinical team in a timely manner
Accurately document patient interactions and care coordination activities
Follow established workflows

Provide on-site support within one or more partnered long-term care facilities
Work alongside Ennoble Care providers in a family-practice-style model of care

How You'll Work.
Team & Collaboration
Collaborate closely with Ennoble Care providers, within the long-term care setting; Serve as a key liaison between providers, facility staff, residents, and families; Contribute positively to team collaboration and facility partnerships; Working alongside providers and facility staff
Communication Scope
Comfortable communicating proactively and professionally throughout the day via Microsoft Teams, email, phone, and in person; Communicate patient needs, changes in condition, and urgent concerns to the clinical team in a timely manner

Ennoble Care is seeking a full-time Care Coordinator, Embedded to join our team, providing on-site support within one or more partnered long-term care facilities in the Reston, Virginia area. This role is an integral, in-person member of our clinical care team, working alongside Ennoble Care providers in a family-practice-style model of care that emphasizes continuity, relationship-building, and patient-centered support.
