Full Job Description
Care Coordinator Full-Time | $19-22 (DOE) At Pathways for Life, you’ll be a part of a mission-driven organization dedicated to helping individuals live meaningful, independent, and fulfilling lives. We are seeking a compassionate, organized, and relationship-driven Care Coordinator to support individuals with developmental disabilities and the caregivers who serve them. As a Care Coordinator, you will play a vital role in ensuring members and families receive consistent, high-quality support while helping caregivers feel connected, supported, and successful. You will serve as the primary point of contact after services begin, helping maintain service stability, resolve concerns, coordinate care, and strengthen member-caregiver relationships. What You’ll Do Serve as the primary point of contact for members and families after services begin. Build strong relationships with members and their support teams. Attend Person-Centered Service Plan (PCSP) meetings and advocate for member needs. Ensure services align with member goals, needs, and authorized support. Address questions, concerns and service issues in a timely and professional manner. Develop and maintain positive relationships with assigned caregivers. Conduct check-ins with new caregivers within their first five days of service. Monitor member-caregiver compatibility and service satisfaction. Coordinate caregiver changes, when necessary, due to scheduling, performance or fit. Support long-term service stability and positive outcomes for both members and caregivers. Monitor authorizations, EVV compliance, timecards, and service documentation. Assist in resolving billing and authorization issues. Support caregiver recruiting, onboarding, and member intake activities as needed. What We’re Looking For Must be at least 18 years old.
