Eligibility Requirements

Lift a minimum of 10 lbs, Pull a minimum of 20 lbs, Squat, kneel, crawl, crouch, Stoop, climb, Stand and walk, Complete criminal background check, Name check in registries, Drug testing, Education verification, Credentialing, Proof of driver's license, Driver's insurance, Vehicle if required

What They're Looking For.

Must Have

Current North Carolina Registered Nurse (RN) license, Two-year or four-year degree, One year of case management experience, Completion of an NC Medicaid-certified training program within 90 days

Nice to Have

Experience with Epic EHR, Experience with Cerner EHR

What You'll Do.

Conduct pre-screening and assessments

Consult with beneficiary and caregiver

Identify needs to prevent health and safety factors

Develop and maintain person-centered care plans

Update care plans at least every 12 months

Ensure beneficiaries and families understand plan of care

Monitor service quality

Arrange for additional services

Monitor services through phone calls and home visits

Evaluate effectiveness of care plans and services

Provide emotional support and counseling

Assist families in navigating challenges

Assist with obtaining documentation from medical staff

Maintain accurate and up-to-date records

Ensure compliance with 10A NCAC 27G.0202

Review and ensure proper billing codes

Participate in NC Medicaid-certified training programs

Ensure timely updates to CAP Business system

Work closely with SWs and interdisciplinary team members

Act as an advocate for program participants

Work closely with service providers

Provide training and support to families

Complete all state-mandated training

Stay current on CAP guidelines
