Must Have

1+ years of general work experience, Physically located in California, Reliable transportation, Quiet, HIPAA compliant and internet connected space, Bilingual – English/Spanish, Understanding of cultural fluency, High degree of empathy, Ability to work collaboratively in a multidisciplinary team, An eye for optimization, Organizational skills, Ability to remain patient when faced with adversity, Comfort with technology innovation, Basic Excel, Basic Word, Basic email, Basic video conferencing, Valid driver’s license, Auto liability insurance

Nice to Have

Demonstrated professional or personal lived experience working closely with individuals experiencing complex chronic needs, homelessness, or Severe Mental Illness/Substance Use Disorder, 2+ years of case management experience, Experience with motivational interviewing, Knowledge of medical terminology, Zest for problem solving, Seeking answers, Thinking outside the box, Detail-oriented, Organized self-starter, Rockstar multitasker, Reliable, Comfortable in an ever-changing environment

What You'll Do.

Maintain ongoing caseload

Promote engagement and achievement

Support members with social support navigation

Conduct telephonic and SMS outreach

Identify health/wellness goals

Incorporate goals into Health Action Plan

Support care managers with delegated tasks

Collaborate on care issues

Participate in systematic case reviews

Consult with care managers

Meet monthly encounter metrics

Assist individuals in securing community supports

Schedule appointments

Ensure timely follow-ups

Coordinate physical care management appointments

Utilize online platforms

How You'll Work.

Team & Collaboration

Work collaboratively in a multidisciplinary team; Collaborates on care issues with Enhanced Care Management team; Consulting with nurse care manager, behavioral health care manager, and nurse practitioner before taking clinical actions; Utilize external and internal online platforms to collaborate with team members
