Must Have
1+ years general work experience, Physically located in California, Valid driver's license, Auto liability insurance
Nice to Have
2+ years case management experience, Experience with motivational interviewing, Knowledge of medical terminology
What You'll Do.
Maintain ongoing caseload
Promote engagement and achievement of health goals
Support members with social support navigation
Conduct periodic telephonic and SMS outreach
Identify health/wellness goals
Incorporate goals into Health Action Plan/Shared Care Plan
Support care managers with delegated tasks
Collaborate on care issues
Participate in systematic case reviews
Consult with care managers
Meet monthly encounter metrics
Identify and break down barriers
Assist individuals in securing community supports
Schedule appointments
Ensure timely follow-ups
Coordinate physical care management appointments
Collaborate with external and internal providers
Utilize online platforms to collaborate
Carry out daily tasks
