Conduct outbound patient outreach calls in both English and Spanish

### Chronic Care Staffing is Hiring a Bilingual Care Coordinator - LPN/LVN! **Location:** Remote (MST or PST Time Zones Only) **Schedule:** Monday–Friday **Pay Rate:** $24–$27/hour **Employment Type:** Full-Time ### Overview Chronic Care Staffing is seeking a compassionate and highly organized Bilingual Care Coordinator - LPN/LVN to support patients managing chronic health conditions through proactive outreach, education, and care coordination. This fully remote opportunity is ideal for bilingual nurses who want to transition from bedside care into a telephonic care management environment while maintaining meaningful patient relationships and improving long-term health outcomes.
