You will focus on enhancing quality of life, reducing behaviours of concern and promoting the rights of people with disability. You will work as part of a team including practitioners, a team leader and clinical advisors, to support people with evidence-based practice along with developing your own knowledge, skills and experience. You live in Chermside or a suburb very close to Chermside. As a Behaviour Support Practitioner you will: * Support our clients (NDIS participants), along with their families and/or carers, to identify their needs through assessment and person-centred planning * Identify meaningful goals and practical strategies utilising The Positive Behaviour Support Capability Framework, with a commitment to eliminate restrictive practices, in the home and community * Develop, deliver and evaluate evidence-based support plans within a complex support space * Ensure individual plans reflect our clients' goals, passions and strengths * Build the knowledge, capacity and strengths of those who support our clients - family, carers, friends, etc Why You’ll Love Working for Outcomes Connect Australia Supervision and Su
