Eliot’s CoCM is an integrated care approach that coordinates behavioral health, substance use, and medical services across primary care and community-based settings to improve access, continuity, and outcomes. The Behavioral Health Services Manager functions as part of the primary care team and supports shared treatment planning, provides evidence-based interventions, assists with care coordination, and monitors patient progress. The ideal candidate demonstrates a strong commitment to mental health recovery, substance use disorder treatment, integrated care coordination, and overall patient wellness. The role requires comfort with providing community-based support, including traveling as needed to deliver services where patients are located.
