Must Have
Three years of experience in value-based care or managing adult patients with chronic medical conditions, Masters' degree as a nurse practitioner from an accredited medical school, Active, autonomous nurse practitioner medical license in the state of practice, or to obtain, Board Certification, Screened for TB, Willing to participate in limited on-call coverage by phone
