**Overview:** **Job Description Summary:** Coordinates clinical program activities, including communication with stakeholders, scheduling, and resource management to ensure successful program implementation. Provides healthcare to individuals or groups that requires knowledge and skill obtained from advanced formal education, certification, and clinical experience. Practices within professional scope, promotes patient advocacy in accordance with the profession’s Code of Ethics, and maintains a current Ohio Advanced license. **Job Description:** **Essential Functions:** * Implements and upholds values and standards while collaborating to develop program goals and best practice guidelines. Utilizes evidence-based practice and serves as a clinical expert for policies procedures, documentation, and skills. * Upholds professional competence to support program operations and provide expert consultation to team members on clinical operations and program growth. * Assesses patients and/or populations, develops, implements, and evaluates plans of care, clearly documents, and communicates information efficiently. * Performs diagnostic and therapeutic procedures according to established guidelines and standards. Leverages technology and informatics to optimize healthcare outcomes. * Assists in the education, training, and development of new and existing staff members.
