What They're Looking For.
Must Have
Physician Assistant or Nurse Practitioner accredited program completion, Active, unrestricted license to practice in Louisiana, 2+ years clinical services experience, Current CPR certification (BLS)
Nice to Have
Experience leading interdisciplinary care team, Experience in startup environment
What You'll Do.
Manage panel of families
Provide longitudinal primary and preventive care
Provide care management
Provide care coordination
Document member interactions
Communicate with team
