# **** SIGN ON BONUS ELIGIBLE **** # Baystate Health, western Massachusetts’s premier healthcare provider, and home to Baystate Medical Center (BMC), a Magnet Hospital for Nursing Excellence is looking for a **full-time Ambulatory Nurse Professional Transplant Program Coordinator!** # **Location:** * **100 Wasson Ave Springfield, MA** **Schedule:** * **Full-time, 40 hours** * **Monday - Friday (8am-430pm)** * **On-Call rotation** Under the direction of nursing and physician leadership, the Transplant Nurse is responsible for coordinating and delivering comprehensive care, education, and support to transplant candidates, living donors, waitlisted patients, and post-transplant recipients in both inpatient and outpatient settings. This role partners with a multidisciplinary team to provide patient-centered care, including assessment, care planning, case management, and education. Responsibilities include facilitating inpatient care coordination, discharge planning, and transitions of care, as well as providing telephone triage and longitudinal patient management throughout the transplant or donation journey. Education is a core component of this role, including patient, family, donor, staff, and community outreach education. The coordinator also contributes to program development, quality improvement, and regulatory compliance. **SUMMARY:** * Coordinate evaluation, work-up, and listing processes for transplant candidates and living donors; manage longitudinal follow-up for post-transplant patients. * Utilize the nursing process to assess, plan, implement, and evaluate individualized patient care across the continuum. * Collaborate with physician leaders and interdisciplinary teams to design and improve care delivery systems. * Demonstrate advanced knowledge of kidney function, chronic kidney disease (CKD), and end-stage renal disease (ESRD) treatment options. * Facilitate multidisciplinary rounds, patient care conferences, and transplant selection committee presentatio
