What They're Looking For.
Must Have
Advanced Practice Licensure (APN) or Physician Assistant (PA) Licensure in the State of IL, Federal DEA and Illinois Controlled Substance License required, Completion of master’s level program in specialty area, BLS/CPR certification required
Nice to Have
Prior APN/PA experience preferred, Experience working in an outpatient family medicine setting, Experience with acute and chronic condition management, Experience with preventative care

**Hourly Pay Range:** $56.44 - $70.58 - The hourly pay rate offered is determined by a candidate's expertise and years of experience, among other factors. **Endeavor Health is continually recruiting nurse practitioners and physician assistants for future full-time and part-time positions in our Primary Care clinics across the Northwest Suburbs of Chicago. Under the direction of the collaborating physician(s), provides care to patients. Obtains detailed history and performs physical examinations, orders imaging, lab tests and diagnostic procedures and interprets results. Instructs patients on the prescribed therapeutic regimen and pre/post operative instructions. Maintains records and writes progress notes on status, treatments and procedures. Follows patients through their course of treatment in the clinic and hospital setting. Collaborates with physicians and the health care team to achieve optimal patient outcomes. Prescribes pharmacologic and non-pharmacologic therapies as appropriate. Demonstrates evidence-based practice. Participates in the development, implementation, and evaluation of standards of practice in the specialty. Assesses the learning needs of patients and families across the continuum.
