Must Have
Master’s degree from an accredited NP Program or PA program, Ability to communicate positively, professionally and effectively with others, Problem-Solving Skills, Effective problem solving, organizational and time management skills, ability to work in a fast-paced environment, Active California state Nurse Practitioner or Physician Assistant license, Active RN License and Furnishing number, Active Nurse Practitioner Board Certification/Physician Assistant Certification, NPI Number, DEA, Valid BLS, Valid California driver license, current automobile insurance
Nice to Have
One (1) year of prior clinical or home care experience, Previous EMR experience, Experience in care of older adult (geriatric) patients
What You'll Do.
Conduct in-home assessments
Obtain comprehensive history
Perform physical exam
Perform cognitive screenings
Perform depression screenings
Perform safety screenings
Collect laboratory specimens
Manage chronic diseases
Communicate patient assessment findings
Provide patient education
Provide family education
Provide caretaker education
Refer to community resources
Comply with HIPAA regulations
Maintain security of PHI
