Our care model combines in-clinic and virtual care to bring support to patients where and when they need it.   Backed by $50M in funding—including a $25M round led by CVS Health Ventures with support from a16z Bio + Health, Flare Capital, MassMutual, and Intermountain Ventures—we’re scaling fast and expanding access to evidence-based obesity care nationwide.   As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health.   Location: Schaumburg; hybrid role Schedule: Monday-Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time).  Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call EMR: Athena On-Call: Phone call only, in a rotation every 7-8 weeks for one (1) week at a time. Average call volume - <1 call per WEEK (as of 8.1.24 http://8.1.0.24/). 24/7/365 physician back up. Requirements: - Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA). - Unrestricted DEA licensure and prescribe authority (per state practice regulation). - At least 2 years of clinical experience in an ambulatory care setting, family o
