We offer weight management, primary care, nutrition counseling, and health coaching. Our care model combines in-clinic and virtual care to bring support to patients where and when they need it. Backed by $50M in funding—including a $25M round led by CVS Health Ventures with support from a16z Bio + Health, Flare Capital, MassMutual, and Intermountain Ventures—we’re scaling fast and expanding access to evidence-based obesity care nationwide. As an Advanced Practice Clinician (APC) at knownwell, you will work alongside a team of committed physicians, APC, registered dietitians, registered nurses, and registered pharmacists, to provide comprehensive medical care, with a specific focus on the diagnosis, treatment, and prevention of obesity and obesity related conditions as appropriate in a primary care setting. This position offers an opportunity to make a significant impact on the lives of patients by helping them improve their overall health. Schedule: Monday- Friday with flexible scheduling, 32 patient contact hours minimum (+ 8hrs of administrative time). Hybrid model of care. No scheduled evening/weekend responsibilities outside of phone On-Call Location: Midtown Manhattan EMR: Athena On-Call: Phone call only, in a rotation TBD frequency. Call volume – very low 24/7/365 physician back up Requirements - Master's degree (MSN) and certification by one of the Nurse Practitioner certifying bodies (ANCC or AANPCB)) as a family nurse practitioner, or Master's degree in Physician Assistant studies and certification by the Physician Assistant certifying body (NCCPA). - Unrestricted DEA licensure and prescribe authority (per state practice regulation). - At least 2 years of clinical experience in an ambulatory care setting, family or internal medicine; experience in chronic condition management and weight manage
