2–5 years of experience in healthcare coordination, 2–5 years of experience in admissions, 2–5 years of experience in patient navigation, 2–5 years of experience in scheduling, 2–5 years of experience in case management, 2–5 years of experience in customer service, 2–5 years of experience in related healthcare roles, Healthcare experience preferred, Home health experience preferred
What You'll Do.
Serve as primary point of contact for patients
Manage caseload through admissions process
Coordinate physician appointments
Coordinate nursing assessments
Coordinate home visits
Coordinate caregiver onboarding
Coordinate required documentation
Establish patient relationships
Provide education on home health program
Support onboarding activities
Attend physician appointments
Advocate for patients during appointments
Facilitate communication between providers
Facilitate communication between patients
Facilitate communication between internal departments
Assist with obtaining signatures
Assist with obtaining documentation
Assist with obtaining orders
Coordinate communication with patients
Coordinate communication with families
Coordinate communication with physician offices
Coordinate communication with Resource Nurses
Coordinate communication with Authorization Specialists
Coordinate communication with Writing department
Coordinate communication with Staffing department
Coordinate communication with operational departments
Assist with obtaining documentation
Assist with uploading documentation
Assist with routing documentation
Assist with tracking documentation
Coordinate Durable Medical Equipment requests
Communicate patient needs to departments
Ensure patients understand equipment usage
Provide courtesy calls to patients
