Must Have
High school diploma or equivalent required, Valid driver's license, Current auto insurance, Dependable personal vehicle, Ability to travel locally, Computer literacy, Comfort utilizing laptops, tablets, EMR systems, Salesforce, Microsoft Office Suite, and operational software platforms
Nice to Have
2–5 years of experience in healthcare coordination, admissions, patient navigation, scheduling, case management, customer service, or related healthcare roles, Healthcare or home health experience
What You'll Do.
Serve as primary point of contact for patients
Manage independent caseload of patients
Coordinate physician appointments
Coordinate nursing assessments
Coordinate home visits
Coordinate caregiver onboarding
Coordinate required documentation
Provide education regarding home health program
Support onboarding activities
Attend physician appointments
Advocate for patients during appointments
Facilitate communication between providers
and internal departments
Assist with obtaining required signatures
Assist with obtaining required documentation
Assist with obtaining required orders
Communicate with patients
Communicate with families
Communicate with physician offices
Communicate with Resource Nurses
Communicate with Authorization Specialists
Communicate with Writing
Communicate with Staffing
Communicate with operational departments
Support continuity of care
Support workflow progression
Assist with obtaining documentation
Assist with uploading documentation
Assist with routing documentation
Assist with tracking documentation
Coordinate Durable Medical Equipment requests
Communicate patient needs to appropriate departments
Ensure patients understand equipment usage
Provide courtesy calls to patients
Provide milestone calls to patients
Provide follow-up calls to patients
Maintain accurate documentation
Maintain workflow updates
Maintain provider information
Maintain patient records
Assist with caregiver onboarding coordination
