Follow-up on NTUCs and patients discharged

Full Job Description
**JOB SUMMARY** Responsible for managing all aspects of the patient referral and admission process. Establishes and maintains positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. **DUTIES & RESPONSIBILITIES** Performs all daily patient referral and admission operations. Ensures compliance with all state and federal admission regulatory requirements. Ensures patients are admitted in accordance with policy. Establishes and maintains positive working relationships with current and potential referral sources. Maintain accurate census with daily updates. Maintain accurate and current Roster. Maintain Daily Stand-up Report and End of Day Report. Run Medicare / Commercial Insurance and obtain prior authorizations when needed. Create and update whiteboard for referrals / admissions. Obtain Orders and H&P for all new referrals. Assist Marketers with set-up of transportation as needed. Fax consents when needed and follow-up to ensure they are completed. Create / generate letters thanking referral sources for referral; PCP and referral source indicating patient’s admission; PCP informing of patient’s discharge. Create and follow a process for follow-up on NTUCs and patients discharged for extended prognosis.
