What You'll Do.

Provide service coordination

Develop overall service plan

Assist in development of support networks

Provide individual and group supportive counseling

Monitor symptoms and medications

Link individual to appropriate services

Monitor development of appropriate behaviors

Teach skills for independent living

Facilitate practice and transfer of skills

Provide support to family

Write Person Centered Plan

Implement Person Centered Plan

Evaluate Person Centered Plan

Revise Person Centered Plan

Develop treatment goals

Develop service plans

Function as member of multi-disciplinary team

Facilitate training of independent living skills

Establish access to community services

Assist individuals in establishing support networks

Assess individual needs daily

Coordinate services with program staff

Coordinate services with inpatient facilities

Coordinate services with detoxification facilities

Establish linkage to residential programs
