What They're Looking For.
Must Have
High School Diploma, Certified Peer Support Specialist (CPSS), Drivers License (Valid), Lived experience and a personal recovery story related to mental illness or substance use
What You'll Do.
Provide individualized services in the community
Promote individual self-determination
Provide expertise and consultation to staff
Carry out duties as ACTT staff member
Participate in comprehensive assessment
Document participant progress
Participate in staff meetings
Contribute to program assessment
Provide direct services to participants
Provide work-related supportive services
Provide ongoing assessment
Assist participants with activities of daily living
Assist participants to find housing
Assist participants with household activities
Assist participants in obtaining financial support
Teach money-management skills
Assist participants in accessing financial services
Help participants to access transportation
Assist participants to have and use primary care physician
Organize and lead social activities
Organize and lead recreational activities
Attend and participate in meetings
Attend and participate in training
Maintain certification in training requirements
Demonstrate knowledge of agency policies
Comply with agency policies
Comply with state statutes
Comply with federal statutes
Follow service definition guidelines
Complete other responsibilities as assigned
