What They're Looking For.
Must Have
Master's degree in Human Services, LCMHC-A, LCSW-A, LMFT-A, Valid Driver's License
Nice to Have
2 Years Full time experience working with adults with severe and persistent mental illness
What You'll Do.
Assist Team Leader in managing clinical and operational aspects
and evaluate team members
Support individuals in skill development
Provide effective oversight and direction
Participate in hiring and training staff
Assume responsibility for staff schedules and coverage
Monitor authorizations and Person Centered Plans
Schedule and ensure 24/7 coverage
Serve as a practicing clinician
Coordinate services with various facilities
Act as liaison with after hours staff
Document participant activities
Develop service plans
Review caseload and problem solve
Assess and monitor service quality
Market the agency and program
Attend community and agency meetings
Monitor medical records management
Attend meetings and training
Comply with agency policies
Implement service definition guidelines
Complete other responsibilities as assigned
Complete team lead responsibilities in absence of team lead
How You'll Work.
Team & Collaboration
Work with other ACTT staff; Coordinate services with program staff, inpatient facilities, and detoxification facilities; Establish linkage to residential programs, self-help programs and community resources; Liaise with program after hours staff; Meet regularly with other Assertive Community Treatment Team members; Develop appropriate referral sources; Maintain positive working relationships within communities served
