What You'll Do.

Coordinate and support annual EHCP reviews

Complete all required documentation accurately

Chair annual review meetings

Collate contributions from staff

Maintain accurate SEND and EHCP records

Ensure systems and documentation remain compliant

Support provision mapping

Track interventions for students with SEND

Monitor funding deadlines

Monitor required actions

Assist with preparation of reports

Assist with evidence preparation

Assist with supporting documentation

Support students with EHCPs / high needs

Provide targeted interventions

Provide direct work with students

Provide regular check-ins

Assist in coordinating support plans

Assist in coordinating One Page Profiles

Support students in developing independence

Support students in developing organisation

Support students in readiness for adulthood

Work collaboratively with teaching staff

Ensure classroom strategies are implemented

Ensure reasonable adjustments are implemented

Support transition arrangements into Sixth Form

Support transition arrangements through Sixth Form

Support transition arrangements beyond Sixth Form

Attend transition visits

Liaise regularly with teachers

Liaise regularly with tutors

Liaise regularly with support staff

Monitor quality of SEND provision

Monitor impact of SEND provision

Assist with coordination of in-class support
