Must Have
Valid driver's license, Sufficient automobile insurance, Acceptable driving record, Access to reliable vehicle, At least 18 years old, Pass and maintain MAP certification, Pass and maintain CPR certification, Pass and maintain First Aid certification
Nice to Have
College degree in human services, Six months experience with individuals with brain injuries, Six months experience with mental health challenges, Six months experience with developmental disabilities
What You'll Do.
Provide person-centered support
Assist with daily living activities
Assist with personal care
Assist with skill development
Promote community integration
Facilitate access to transportation
Facilitate access to social activities
Facilitate access to external resources
Support participants' health needs
Administer medications
Schedule appointments
Monitor overall wellbeing
Assist with financial skills
Assist with money management
Maintain documentation of expenditures
Respond to emergencies
Respond to crisis situations
Communicate with supervisors
Advocate for participants
Help build self-advocacy
Serve as liaison with families
Serve as liaison with providers
Serve as liaison with community supports
Contribute to ISP development
Contribute to ISP implementation
Document participation
Maintain accurate documentation
Maintain timely documentation
Complete incident reports
Complete health records
Promote a safe environment
Follow safety procedures
Assist with emergency drills
Identify potential hazards
